Respond to Inspection Findings

When an Assignee responds to a Finding, the EHS system defaults to the Findings tab on the "Respond to Findings" page. In order to resolve the Finding, the Assignee must view the Finding, complete the form, save, and submit the report.
Users can get to the "Respond To Findings" form from multiple places within the EHS system. You can:

  • Click on a Finding from the Inspection Task List,
  • Click on a Finding from the "View Findings Assigned to Me" page,
  • Click the link in the Finding Notification email,
  • Click the link from the emailed Level II Inspection Report,
  • Select a Finding from the Inspection Overview page by clicking on a submitted inspection.

The first Finding is always expanded for the user. Additional Finding details are collapsed and will need to be expanded by clicking the arrow.



There are basically three different Finding scenarios that may be encountered when completing the Findings form:

Scenario One: General Findings - The Finding is NOT a DCR (Documented Closure Required)

For Findings that fall under the General Findings category, (those that do not have DCR checked when the Finding is submitted), the Action Taken field shows YES and the Action Description field is automatically completed by the EHS system.



The Action Description field may be modified, if necessary, but the remainder of the form is read-only. To resolve the Finding, the Assignee must first save the form by clicking the Save button, and then submit the Finding by clicking the Submit button.
On the Inspection Overview page, the Findings Resolved status is shown on the Findings table for the Roomset.


Scenario Two: DCR is checked when the Finding was submitted and the Assignee is responding to the Finding

In this case, the Action Taken and the Action Description fields are blank and editable. The Assignee has the choice of checking the Action Taken checkbox or leaving it unchecked. If the assignee enters text in the Action Description text box but the Action Taken checkbox is left unchecked for a DCR finding, the finding is still listed as Open in the system.



If these fields are left blank, then the DCR Finding is carried over when the next inspection is created. When the Assignee goes to respond to the Finding at a later date, the form will be similar to the following.



To clear the Finding, the Assignee needs to click the "here" link (highlighted in red in the example above) to open the current Inspection for the Finding. Once on the current Inspection, to resolve the Finding, the Assignee must complete both the Action Taken and the Action Description fields, save, and submit.

Scenario Three: The Finding is a DCR Carry Over

If the Finding is not resolved on the current Inspection, it is carried over to the next Inspection and may exist on multiple Inspections until the Finding is resolved. In order to resolve the Finding, the Finding must be resolved in the EHS system on the most recent Inspection submitted.
The Findings form will show a notice about the Carry Over, and the Action Taken and the Action Description fields are both editable. All previous Inspection Findings forms are read-only with a notice on the current Inspection about the unresolved Finding.



The following fields are available for selection.
Correction Status: Choose one:
Not Corrected: The Finding has not been corrected.
Corrected During Inspection: The Finding was corrected during the Inspection.
Corrected Prior to Inspection: Displays only if the Finding is a Carry Over DCR. For a first time DCR, this option will not appear.
Exclude from PI Report check-box: Check if you want to keep the Finding out of the PI Report. Uncheck to include the Finding in the PI Report.
Subject to Restriction check-box: This is only seen by DLC EHS Coordinators and DLC EHS Lead Contacts.
Assigned To: Choose who to assign the Finding to from the drop-down list.
